Interesting little quiz. My score was lower than I would have liked but not embarrassingly low. Apparently I make a good citizen.....mostly because I think I'm good at guessing answers correctly

The form question was easy, they asked about what form to "apply" (key word). When you apply for something you fill out an application. There's only one form that's an application in the answers.
